In episode 8 of season 3 of Botched, the doctor duo take on new patients with specific problems. One wants to look like a cartoon while one patient has botched buttock implants and another has had to live with two huge masses on his ears.
Pixee, a 25-year-old woman from Sweden has had numerous surgeries including a brow lift, eyelid surgery, nose jobs, fillers in her face, four breast augmentations and six ribs removed to give her an hourglass look. She says all the plastic surgery she has had added up to $135,000. While meeting Pixee, Botched doctors, Dr. Paul Nassif and Dr. Terry Dubrow notice her the second she walks in the consultation room. Nassif says her removal of six ribs is too extreme. Pixee says her ultimate goal is to look like a cartoon. When the doctors ask her how she wants to achieve her goal, she says she is working with Justin from season 1. Justin was a patient who had invented his own implants. She says the two are working on constructing silicone buttock and hip implants.

During the consultation, the Botched doctors examine her and Nassif says she would look unattractive if she got the silicone implants. Dubrow added that she could also face a lot of health problems. The doctors will not perform any surgeries on Pixee and said they hope she will consider not getting the implants. During a confessional, Pixee says the doctors will not stop her from getting her future surgeries.
Trevor, a young man with two huge masses on his ears, got his ear pierced when he was younger. Later, the mass got larger and he formed a keloid, an abnormal scar tissue, on his ear. He had surgically cut it off, but noticed it was coming back. For the second time, he got it removed by an oral surgeon. He decided to get his other ear pierced but keloids formed on his ears again. Because of the large masses, he says he did not want to leave the house and could not get a job around people. During his consultation, the doctors look at it and see his right ear can have most of the keloid removed but the keloid on his left is more intertwined with his ear lobe.
The doctors say there is a 65 percent chance of it reoccurring if they just cut it off like previously before. Instead, Nassif will remove the keloid masses and use his leftover skin to reconstruct the ear. After revising his reconstruction plan, the surgery is a success. During his check-in, Nassif says his ears are looking good but Nassif says he wishes he could do better. Trevor has to take care of his ears in the meantime. After no longer having any keloid masses on his ears, he meets with his mom and dad after having a job interview to work in the front of a restaurant. He says he is excited to interact with people and thank Nassif for giving him a new chance at life.

Misty, a woman who had buttock implants, worked at a hotel for 11 years and had to have a specific look. The buttock implants she had were high and not symmetrical. To try to fix the problem, she had a second surgery. However, the doctor used the same implant and still did not work, which the Botched doctors says was malpractice and risked Misty have an infection. She says she hopes she can have the doctors remove the implant or fix it. The Botched doctors say her incision is 7-inches long, but should be 1-inch. Misty says she would rather have the pain gone and decides to remove the implants.
Dubrow says he will reduce the scar and take the implants out, along with the scar tissue that could cause a lot of pain. During the surgery, Dubrow wants to try to give her an internal buttock lift to rid the excess hanging skin rather than an external lift. After the surgery, Dubrow says her buttocks is even and should be pain-free. During the check-in, Misty says she misses her implants and wants them back. Dubrow encourages her to wait a couple weeks to see if she heals properly and they explore the possibility of having the implants again.

After a few weeks, Misty talks with the Botched doctors and says she still desperately wants her implants back in. The doctors tell her all the complications she previously had would come back and Dubrow says they will not put implants in. Misty says she appreciates the doctors respect and truthfulness and is excited for her future.
